Once assigned to an approved clinical placement, please complete the clinical site-specific requirements for the applicable clinical site prior to the start of the clinical experience. Students should satisfy and submit clinical site-specific requirements to your clinical faculty or per the advised instructions in ACEMAPP. If students have questions, please contact your clinical faculty. Faculty should submit clinical site-specific requirements of your clinical group to the clinical site contact per the advised instructions. If faculty have questions, please contact your clinical coordinator. Adventist Medical Center

  1. Coordinators: Download and complete the Student Access Request Form. Once completed, please email the form per the instructions.
  2. Faculty review Nursing Instructor Guidelines in the model for student nurse electronic charting education.
  3. Students / Faculty: Complete the Adventist B.L.U.E Book Test and Confidentiality Agreement
  4. Students / Faculty: Complete the Instructions and the Nova Glucose Monitoring Instruction Video
  5. Students / Faculty: Review the AMC Parking Information

Kaiser Sunnyside Medical Center and Westside Medical Center

Upload all documents in ACEMAPP through your clinical requirements list.

  1. Coordinators: Submit the KSMC Student Access Form and KWMC Student Access form at least 3 weeks prior to start date and include faculty if you wish for them to have Epic access. Please carefully read and review this form. Provide all information asked, including student middle initial and context if the student is or was a KP employee. If this information is not accurately reported your student may not have access, access may be denied, or access may be delayed
  2. Students / Faculty: Review the Student Orientation Course and complete the Orientation Exam in ACEMAPP, this will be available in the assessments section of your profile once you are assigned to an approved clinical placement
  3. Students / Faculty: Review the KSMC Parking Map or the KWMC Parking Map (Students and Faculty must park in employee designated spots only, cars may be subject to tow if in member parking)
  4. Students / Faculty: Complete the Confidentiality Agreement, this is an e-signature form that will be assigned to your profile once you are attached to a Kaiser rotation.
  5. Students / Faculty: Review HIPAA Quick Reference
  6. Students / Faculty: Review the Student Placement Policy, this will be assigned to your profile once you are attached to a Kaiser rotation.
  7. Students: Complete the Glucometer Training online module, create a barcode and complete in person Glucometer Training (receive this information from instructor and or coordinator)
  8. Students: please complete HealthConnect learning modules (instructions on logging into KP HealthConnect;  learning module instructions) and upload attestation of completion within ACEMAPP (attestation is good for 12 months). Must be logged on a Kaiser network computer.
  9. Students: Those on Dedicated Education Units (DEUs): Complete a DEU Student Profile on a Word document with the following information and upload within ACEMAPP: 1) Personal photo with head and shoulders only; 2) Background information: About, Interests; 3) Why did you choose nursing? and 4) What are your personal and professional goals?

Legacy Health System

All students and faculty must complete these education modules annually prior to a clinical placement at a Legacy Health clinical site. 

  1. Students/ Faulty: Prior to first clinical experience, read Legacy policies Clinical Experience for Student Nurses, Allied Health Occupation Students, Paramedic or EMT Students, and Medication Administration. 
  2. Students / Faculty: Read Welcome to Legacy Student and Staff Expectations PowerPoint presentation.
  3. Students / Faculty: Sign the Legacy Health Nondisclosure Agreement. Witness signature should be clinical instructor or school designee. Update as a post-assignment clinical requirement in ACEMAPP.
  4. Students / Faculty: Review the Stat Strip Glucometer Competency prior to start of clinical rotation. Faculty member to be trained by hospital super user and then faculty member will be expected to train students assigned to their cohort, StatStrip Competency Checklist. Complete the self-learning module and upload StatStrip Competency Certificate as a post-assignment clinical requirement in ACEMAPP.
  5. Students / Faculty: Complete EPIC Training. Training MUST be completed before you begin your clinical placement. Contact your school's student placement coordinator to receive your login ID. All password changes must first be made at, once successfully updated, you will be able to sign into E+. All new Legacy accounts require the password to be changed before logging in. Epic Training needs to be completed one time only.
  6. Pyxis MedStation Registraion (link).  Login ID for Pyxis is the same as assigned for Epic. Initial Pyxis password will be the same password that user has set-up for Epic. On first login, Pyxis will direct you to set-up fingerprint Bio-ID.
    • If you are unable to login to Pyxis verify that the CAPS LOCK on the keyboard is turned off; the indicator light is small and easy to miss.
  7. Students / Faculty: Complete the Legacy ID Badge Request Form. Form must be signed by authorized school personnel. Take to any Legacy security office to receive badge.
  8. Students / Faculty: Read Legacy Care Communication Policy
  9. School Placement Coordinator: Complete the Student Access Spreadsheet at least 30 days prior to start of clinical rotation. Email to Deb Henry and Legacy Security. Email address for both are included on the document.
  10. Students / Faculty: Read the Married State Preceptorship Model.
  11. Students / Faculty: Assigned to Legacy Randall Children's Hospital: Read Expectations for Nursing Students.
  12. Campus Site Maps: Emanuel/Randall Children's Hospital, Good Samaritan, Meridian Park, Mt. Hood, Salmon Creek

To obtain an ID Badge, please go to Room 3255, on the 3rd Floor of Sam Jackson Hall (directly across the street from the main hospital; see a (Map) to complete a Student ID Badge Request Form.

Student Nurse Website (Guidelines, Resources, Information)

Coordinators: An email will be distributed by the regional education office by academic period advising how to submit requests for nursing program precept clinical placements. Cohort clinical placement requests are considered through ACEMAPP. For cohort clinical placements, contact the specific Providence Clinical Coordinators.

Students: An Orientation Letter will be emailed to you directly advising you to complete the Student Forms Packet. Submit the packet directly to Providence as indicated in the packet and provide a copy of the email clearance statement to your coordinator or designated SON contact. 

Faculty: Review the Orientation Letter and HRO Toolbox. Review the School of Nursing Clinical Requirements Policy and complete the Student Nurse Safety Orientation Checklist with your students prior to the clinical assignment, then have the faculty or staff on the unit sign and return to your school for record purposes.

VA Portland Health Care System (VAPORHCS)

Important: All completed student application packets must be received by the VAPORHCS Education Department's OAA Technician a minimum of 8 weeks prior to clinical start date. After the end of the precept clinical experience, all students need to take the VA Learner Perception Survey at least once a year and only ONCE in the academic year.

Coordinators: Confirm that course descriptions and objectives are included for each course in ACEMAPP and submit the School Faculty Intake Form at least 3 weeks prior to the clinical start date for Electronic Medical Record (EMR) as well as Pyxis MedStation access clearance.
